odt file containing the problem file : Form to be filled in (already filled)
When I try to "Save As " for document in attach , OpenOffice CRASHES and does
not save anything except for save as .txt and Export as PDF (openoffice format
also working)
When saving in .txt , all fields are empty

Ok for now, but :
1. First time filling form and save is ok
2. Again load ,modify some value and save once in odt and once in doc format
3. Reload and what happens :
a. odt file has the modified values in fields
b. doc file has ORIGINAL values in fields
4. If saved in txt format : fields are empty
Maybe it must be a new report, I don't know

No solved : documents has ALL FIELDS EMPTY when saved in .DOC format or other
format, only open office native format save OK.
Try it yourself , take the .DOC file, modify it and save it in DOC format and
see.

I see nothing on what is the prtoblem so again :
The problem is : file can only be saved in witer or pdf format
Try to save in DOC format cause crsh of openoffice
Saving in TXT format = fields are all empty
And DOC format is needed

new attachment : file from club never filled
Fill in some fields , save it as odt,doc,txt,pdf....
Only odt and pdf saved files are correct

For me, must be merged with issue 65951
In spite of common misconceptions, OpenOffice is not an MS-Office clone. The MS
formats are proprietary and the AOO import/export filters for these have been
arrived at by trial and error, so there may be (read: frequently is) some
formatting loss, which is why you are warned on selecting a Save format that is
nor ODF. As you wrote, documents saved in AOO's native formats play well in
AOO.

Sample form document
This is related to your document.
With the attached document, I don't reproduce.
I can save it as .doc without crash.
Also, try to simplify your form to isolate the problem.

My debug-build gives on save as ".doc" the error messages:
