This has been addressed as part of the patch submitted for http://issues.apache.org/jira/browse/AXIS2-837.

"Now there are two more options 'efd' and 'afd' for setting the elementFormDefault and attributeFormDefault values to the xmlschemas. These will default to 'qualified' when none of these options are specified. "
