I've added dynamicParameters collection to {{SqlString}}, so {{JdbcToEnumerableConverter}} can know which dynamic parameters to use. See https://github.com/apache/calcite/pull/902
{{PreparedStatement.set...}} is not there, yet I'm open if anybody would like to chime in and implement it.
