Remove {{index.html}} from Traffic Router. There seems to be some legacy reason that this was included in the project that is now unknown, so we need to remove it to allow people to route on any file in the request path coming into Traffic Router. The exceptions to this are {{clientaccesspolicy.xml}} and {{crossdomain.xml}}.
