Document that crashes OO Writer
When trying to open the attached file OpenOffice Writer crashes every time. I
have tried to re-install OO and have been able to open other files
successfully, but this one crashes it every time I try opening it.

Clearly, you have a problem install.
By example, setup has not been execute with administrator rights.

What version of Windows are you using? I opened the file on Windows XP-SP3 with
AOO 4.1.1 with no problem.
