Adding a header and footer removes normal text for header and footer.

HeaderName and ReadmeName removes the normal text for the header and footer -
such as "Index of /docs". You should be able to supplement this text, without
having it removed. Otherwise you have to run a shell script grabbing
REQUEST_URI, etc to get this data. You should be able to, perhaps, put in a
directive such as HeaderSupplement to add a supplemental listing after the
header as opposed to removing it altogether.
