This is a known problem with various versions of IEx, which requests
the PDF two or three times from the servlet. There is nothing FOP
could do about it.
It is recommended to use a URL ending in .pdf for requests delivering
PDFs. It sometimes helps to append a dummy parameter dummy=.pdf to the
URL.
