Have you looked at DocBook (http://www.oasis-open.org/docbook/)? Your document
definition looks pretty similar with it. DocBook seems to be a fairly used
system in publishing, although I have no experience with it.

I have looked at docbook, and it is a complex schema.  For the purpose of web
publishing, it is best to use a simplified version of it.  The XSL system
that Norman Walsh (http://www.nwalsh.com) came up with is feature complete,
but it takes a few seconds just for Xalan to compile the thing.
