Please include jai_imageio in your classpath. However... you won't be happy, as your PDF will be rendered with artefacts. Solution: see my comment of 22.10.2013 in PDFBOX-1752. That one works with your file (and many others with the same problem), but there's one other file it can't render (I can't find which one it was).
I'm making a small change so that users get a meaningful error message.
