Namespace prefix on accessors

Even if it is allowed to have the accessors be non-namespace elements, I
think to
be in compliance with the XML Namespace standard we need to allow a
namespace
if given (this I think is true for any accessor..which we also don't handle
I don't
think).

I've been pouring over the doc:
