The table header borders are not adjusted to the borders of body cells

When I create a table which has border around its header and border around
individual cells I see the following issues:
1) borders between the header and body cells are not collapsed, so I have double
width border
2) vertical borders of the header and cells are not adjusted. Borders of the
header is placed a little bit wider than borders of the cells
