Problem with rogue child processes

I am having a problem with apache 1.3.20 on unix.  The
server creates a number of child process, as expected,
however some of these child processes spawn children
of their own.  Where this happens the original child
process becomes stuck in state 'W' (sending reply)
whilst it's child is waiting on a lwp_mutex_lock.

This situation then persists indefinitely.

Can anyone explain to me what is going on here?
