This is probably just something that hasn't been implemented yet but I figured
I'd submit this anyway. If nothing else the attached file could be a test case.

Accorinding to the FO spec a fo:table-column is optional in a fo:table but FOP
seems to ignore tables/columns that don't use fo:table-column.
