I have seen something similar when IE downloads a PDF file over https and
either a 'Cache-Control: no-cache' or a 'Pragma: no-cache' header is sent by
the server. I guess it takes this combination as an instruction never to
write the file to disk for any reason, and so is unable to write it out for
Acrobat Reader.
