FYI, this bug is still occurring for me in struts 2.0.14, its behavior is unchanged since 2.0.11.  I've also tried the 2.1.x branch and default-action-ref does work then.

For what its worth, I'm running linux, and have tried my code out on Ubuntu and CentOS.  

My java version is: (on both computers)

java version "1.6.0_13"
Java(TM) SE Runtime Environment (build 1.6.0_13-b03)
Java HotSpot(TM) 64-Bit Server VM (build 11.3-b02, mixed mode)

My tomcat version is 6.0.18.
