Saving a filled-out form on OpenOffice...... HELP

I've created some forms using the Form Control tool. After filling out the
form, which contains text & check boxes, I can save it BUT it saves a blank
form..or at least opens up that way.

We're using these forms for our insurance company and would really
appreciate some help / feedback. Please get back to me as soon as possible.
