For the record, I was able to reproduce with my first multiplexer run here: https://cassci.datastax.com/view/Parameterized/job/parameterized_dtest_multiplexer/190/

However, after I added additional logging, I could no longer reproduce, despite many multiplexer runs like this one: https://cassci.datastax.com/view/Parameterized/job/parameterized_dtest_multiplexer/217/

I'll try to drop the logging back down to the original level and see if I can selectively add logging without making it non-reproduceable.
