This fix requires the soap:fault element to have a name attribute matching the containing wsdl:fault element name attribute. This means we'll only accept BP-compliant WSDLs, which AFAIK is not the Axis2 goal.

Note that only the BP requires the soap:fault element to have a name attribute.
