Performance branch: Membuffer cache review

Hi devs,

To aid the performance branch review process,
I've extracted the change that probably gives the
largest performance gain. The changes were
grouped in a way that eliminated all possibly
confusing temporary code.

So, without any further ado, please review:

  /branches/integrate-cache-membuffer
  (revisions r998649, r998843 and 998852)
