Since the last append the serializer code has been seriously re-worked.

The serializer is now a fusion of the old one used by Xalan-J interpretive and 
the old one used by XSLTC.  Chances are high that the behavior descibed in this 
bug has changed. In addtion bugzilla 19591 was recently fixed which is related 
to the DOCTYPE not coming out.

Please confirm that this one is still a problem, or we will be shutting this 
bug down.
