1. The bug was introduced in a refactor of the way the TLP servers are deployed using puppet. It wasn't until the DNS changes went live (02:00 UTC) that the issue would have been noticed.

2. There is not a formal post-mortem planned, but we will be discussing it (as well working on a test environment to eliminate issues like these).
