   This is the start of my and @flimzy's "ditch the MoinMoin wiki" project. This effort is approximately _infinite_ years overdue.
   
   The guiding principles are to:
   * add new content to our docs
   * rewrite advice on the wiki to avoid copyright/contributor problems, and
   * to remove pages from the wiki as these PRs land.
   
   A lot of the information is "best practices" info, so I've added an entire new chapter for Best Practices.
   
   This is the https://wiki.apache.org/couchdb/Working_with_Forms page, condensed to avoid redundancy.
   
   Some of the information on the page is generally useful as JS development tips, so I started a new section for that as well.
