Lenya 2.0 and a Page Styled by *Only* CSS

Lenya Users,

I am still struggling to import an existing website
into Lenya 2.0. As I've mentioned, this site consists
of XML files and a custom schema. Using 'make' we
output corresponding XHTML (strict), and then style
the site with CSS exclusively. 

Given:

    index.xml (XML source file)
    index.xsl (index.xml -> index.xhtml)
    index.css (CSS to style index.xhtml)

can anyone tell me where these files need to be
placed, and what configuration files need to be
modified to deploy this presumably common pattern?

I have read all available documentation, and spent
quite some time experimenting, but have had no
success.  Given that we've passed the 'difficult'
stage, and are now successfully maintaining content in
pure XML, and styling with CSS, it's rather fustrating
not to be able to import this into Lenya, especially
since it appears designed to do exactly what we want!
