Please sanity check the 10.2 beta for ASF source header updates

The relevant Jira issues are:

The due date for this change was moved out from Sep 1, likely to
November 1 [1], but we'll see what Cliff ends up emailing to
committers@.  This means that changes aren't strictly required for the
Derby 10.2 release unless it gets delayed past Nov 1 (shudder), but most
of the work has already been completed with the exception of the following:

  - DERBY-1377 : sql files
  - DERBY-1565 : the html files that comprise the manuals

Please post if you spot anything that doesn't look right, such as any
copyright statements in the source files.
