Resolution: Incomplete
Prashant,
Please provide steps to reproduce this bug.
e.g.
(1) log in cloudstack UI.
(2) click Storage menu.
(3) click "Add Volume" button

However, the bug was filed more than 1 month ago and might be fixed already.
If you are able to reproduce this bug based on newest source code, please provide database dump again.
(Since the database dump you attached 1 month ago is out-of-date)
