Screenshot illustrating issue
choose Tools > Customize... , dialog "Customize", tab page "Toolbars"
In case of a custom Toolbar both action:
- to change Toolbar name: Toolbar > Rename...
- to change Toolbar command name: Modify > Rename...
display dialog with SAME title i.e. "Rename Toolbar".
Maybe dialog for renaming toolbar command should have title "Rename Toolbar
command" or "Rename Toolbar item"?

Unfortunately Pootle requires a registration (at the moment).
One note:
Translations done on Pootle will be for 4.2.0.
However, if you happen to use Windows, you can always get my test builds here:
