My bad: I'm using OS X's preview to view PDFs, which lets you add bookmarks from the Bookmarks menu.

I mistakenly assumed this recorded bookmarks into the PDF, but on testing it looks like the PDF itself is NOT modified and the Preview app is storing these bookmarks, globally, elsewhere.
