    Bug: import-mbox.py keeps rechecking if the index exists

    The BulkThread class is responsible for doing bulk inserts into the mbox and mbox_source doc types.
    
    However it starts by checking if the main index (e.g. ponymail) exists, and if not, it creates it.
    
    This results in one index check for every 20 messages on average, whereas at most one check is needed.
    
    Furthermore, if the index is created, it creates a bare index, i.e. without setting up any mappings.
    This can cause problems later on if the auto-created mappings are not suitable.
    
    The code should check once if the index exists, exit with an error if not.
