yes, it is by design.

-r doesn't exit(1)/exit(0) on spam/nonspam.  I can't reproduce what you're
reporting there.   But -c does, by design:

  -c                  Just print the summary line and set an exit code.

So currently there's no way to get *both* a report and the exit code, or *both*
a marked-up mail and the exit code.  that would be an RFE, and we'd need a patch...
