Version 1 of Form.js doesn't support the creation of a form with a DOM Element.
This functionality was implemented with v2 but that version (in return) lacks
some functionality of v1 (eg. passing of bizdata, which is an important one). In
time there should be one version merged from the different current ones, and
this may be one step further in that direction.

Thus v1 should be extended with the functionality to create a form starting from
a DOM Element (as in v2). A patch will be attached which incorporates this
change. (while looking at the patch, the third option in DefaultFormManager:
create form from source, is also available now).
