I think it sounds like race conditions in the Berkeley DB code, partly.

But I still stick with the advice re: using on a soft-mounted NFS filesystem;
NFS is just not up to the task at all, and that's exactly what the Berkeley DB
people state. (and btw if the locks are on the NFS fs, then the dbs are too,
bayes locking doesn't keep locks on another fs.)

If you need to use NFS, I suggest keeping the bayes dbs on a local fs, or
switching bayes off.
