APR design doc.

I have just committed an APR design document.  This is by no means an
authoritative design doc.  It is really just a set of rules that the
current APR developers have been following, to make things look the same.
This is also not a style guide.  For a style guide, we are using the
Apache style guide.  Please fell free to comment on the design doc.
