Fixed jira AXIS2C-220

Hi,

Module refs in service.xml file fails to engage. Instead server start 
returns without success.

I fixed this problem. I tested this with rampart by adding a module ref 
module.xml of rampart.
I also added the module to a user phase. Then server starts successfully 
and module is
enagaged.
