[BOOK] HTML Help files

Hi,

attached is a HTML Help project and contents file to generate
a HTML Help version of the book:

  hhc svn.hhp

It should be possible to autogenerate these files, but I'm not
familiar with the source files of the book and the build process.
Anyway, maybe it's useful as a starting point.
