Create separate application sample for binding-echo

The binding-echo sample contains a mixture of application and extension code. The appllication sample code is incorrectly placed in src/test (it should be in src/main). To follow our agreed conventions on code structure for samples, the application code needs to be separated into its own sample and placed in src/main.
