new UI for callbacks

We've been brainstorming improving the callbacks mapping UI. Currently it has usability problems - you don't see the big picture of all callbacks for a given entity. So experimenting with a flat list instead:

This is not the final UI of course. E.g. we might use more attribute-like UI, with delete button on a toolbar instead of every row. The main idea is a flat list of all callbacks.
