The add/edit screen is missing the red "X" for removing of patterns.
Looking at the browser side "view source" you can see <img src=""> which is an empty image.
What needs to occur.
* The <img src=""> needs to be corrected to actually include the URL to the image/icon.
* The <img> tag needs to include an alt="[X]" tag.
This bug exists for blacklists, whitelists, and patterns.
