Apache NT

OK, I've tagged the head as APACHE_1_3_PRE_NT.

Here goes with the changes... there's still work to be done, and I've hardly
tested it at all, but I'm running out of time...

Please review the changes carefully - I think I've eliminated out-of-dateness
and other strangenesses, but I may have missed something.

Do people want me to post the patch when I'm done?
