This patch fixes the issue when the attribute form="unqualified" is specified on the inner-element.

Trying to come up with a patch that fixes the issue when the form of the element is specified on the schema alone:
<xs:schema elementFormDefault="unqualified" 

Please let me know if you expect to fix this bug directly on the XmlSchema API so that getQName() takes ointo account the XmlSchemaForm directly.
