SecondaryIndexManager.setIndexRemoved(Collection<ByteBuffer>) method is broken, it calls SecondaryIndex.setIndexBuilt(ByteBuffer) instead of using SecondaryIndexManager.removeIndexedColumn(ByteBuffer).
