[C++] PHP Extension

I've spent quite a lot of time just trying to get the extension back up and
running again. I've now back to where I was with the very basic functions
working. I've created new patches (
http://issues.apache.org/jira/browse/TUSCANY-974). The README's are very
thin so I will be adding more in the coming days.
