It's a duplicate of half of DERBY-502 -- that bug complains both about "Web Sample" in the index.html files and about "XSL FO Document" in the HTML book files. Each of these is a different problem -- one that I don't even know enough to figure out if it can be solved. It might make sense to separate DERBY-502 into two different bugs.
