[PATCH] Fixup env vars in server-parsed ReadmeName and HeaderName files

This patch builds on a earlier patch, "Subject: [PATCH] ReadmeName and
HeaderName files server-parsed and content negotiated". This patch fixes
the environment variables in the included files, when they are server-parsed
by mod_include, to reflect the directory's information, not the included
file's information.
