I tried to reproduce this with one of my sample. But the above mentioned scenario is working fine for me.

I tested three, four and even five different input parameter methods in here. All working fine. I'm attaching the sample I used here.

The URL form which takes five different input parameter in this sample will be like : 

http://localhost:8080/axis2/services/StudentService/addSubjects?name=kishanthan&s1=chemistry&s2=physics&s3=mathematics&s4=statistics

The relevant schema generated for this is
