<servlet-name>jsp.index_jsp</servlet-name>
<url-pattern>/</url-pattern>
</servlet-mapping>
until jetty fixes the default servlet.
This showed up because I deleted the uncompiled jsps.

re-included the jsps since jetty may not fix this soon and the servlet mapping doesn't seem to actually work reliably. rev 750118.
