[Patch] another cause why Cocoon may not start

Hello:

I've created a diff file against faq.xml to add another cause why cocoon may
fail to start. If you're using the pja classes as described on the
installation guide, Tomcat will start and display the index.html page, but
will hang when trying to access cocoon
