test(nameserver): pin the upgrade request contract

UpgradeNameServerDTO carries a NameServer upgrade request. Three tests
pin: a valid request passes with the version reported separately from the
address; every field of the upgrade key (cluster, address, target
version) is required; and a blank field counts as missing.
Discriminating mutations (build result verified): the targetVersion
constraint message drifted (red). Restored: 3/3 green.
