If the content is served from the cache (mod_disk_cache), it doesn't show up on
the access.log. If the content is not cache, I can see it.

Not true for me.
If you are still having this problem, please take it first to the
users@httpd.apache.org mailing list where we can see if it is a config error or
a real bug.
