FYI. authentication handler working with apache reverse proxy.

Hi all,

It's FYI, but your comments are always welcome.

I read the article; http://wiki.apache.org/couchdb/Apache_As_a_Reverse_Proxy
But it doesn't work with the authenticate_db, so I wrote an authentication handler.

It sets the user's role to userCtx  by the authenticated username by apache.

I don't know anybody else needs this feature, but it works well to my purpose.
