Problem upgrading from Cloudstack 4.1.1 to 4.2.1

I am trying to upgrade to Cloudstack 4.2.1 but the database upgrade process
is failing with the following error.

Has anyone run into this?
I had originally upgraded from 4.0.1 to 4.1.1 and that went fine several
months ago.
